The Meadows is a Community Based Residential Facility (CBRF) open in Sparta.
(CBRF is a place where five or more unrelated people live together in a community setting.)

The Meadows has been converted from a wing of the Rolling Hills Nursing Home (Haven House) to a Community Based Residential Facility (CBRF). The Meadows is a place where care is offered for the Tenantís emotional and physical needs. The Meadows goals are to preserve the Tenantís dignity while providing a safe living environment, as well as providing quality care to individuals within a home-like setting. The Meadows will encourage Tenants to continue to make as much daily decision making as possible. This will assist to maintain Tenantsí health, well-being, dignity, independence and safety.



Admission Criteria:
The Meadows will admit only the applicants who meet the following criteria:
Attainment of the minimum age of 55 years and certified as free from communicable disease by a licensed physician.
The applicant must possess the basic self-help skills and must be semi-independent in feeding, bathing, toileting and dressing.
Absence of acute medical or physical issues which would require a greater degree of care than would be provided in a CBRF.
Absence of behavioral problems, the result of which would tend to endanger the tenant or others.
The applicant must be capable of taking medication under the regulations of DHS 83.
The applicant must be able to routinely participate in and benefit from the services provided by
  The Meadows and have the ability to live within The Meadowsí house rules.



The Meadows, CBRF Health Monitoring Service:
ADLís (activities of daily living) per tenant need as determined by the pre-admission assessment
B/P (blood pressure) and weight monitoring
Accu-checks on diabetics (blood sugar monitoring)
Staff will assist with tenant medications in accordance to the current medication ruling outlined in DHS 83.33(3) (a-j)
Charting will be done on a daily basis in order to keep staff updated on all tenantís conditions and to assist in accurate implementation of the Individualized Service Plan
All medications will be kept in a secured locked storage area of the facility unless is deemed in the best interest of a tenant to manage his / her medications more independently in which case they can be kept in a locked drawer of the tenantís room with prior assessment and approval by the facility.
The Coordinator will also see that a follow-up health assessment is completed for each resident and that a written order is on file for any necessary medical services.

Community Connection
Bulletin boards will hold calendars of events and announcements concerning programs and services of interest to the tenants.
Radio, TV and music entertainment are available to the tenants of The Meadows as well as table games, magazines, puzzles, and newspapers.
The staff will be encouraged to make use of community recreational opportunities. Staff will actively promote tenant participation in a program of
weekly activities. Community events of interest to the tenants will also be provided at the tenantís expense. Local beauty and barber shops will
be used to help keep the tenant active in the community. There is also a beautician located in the attached Rolling Hills complex that can be utilized
by tenants if desired. The Meadow also works with community organizations and individuals for additional leisure services.
